Fallout 76 Phantom Map Bug Fix: Make the Damn Map Work

If your next Pint-Sized Phantom Map refuses to activate, the most reliable workaround right now is still the ugly one: change servers and try again.
There is, however, a better method players are now independently getting results with. Clear your old Phantom Maps, earn a fresh one, use it immediately, finish the grave, then repeat the whole thing one map at a time.
Basically, don't let them pile up.
Nobody has actually proven that stacked maps are causing the bug, so I'm not going to confidently accuse your inventory of criminal conspiracy because three Reddit threads looked at it funny. What we do know is that players are hitting the same problem, and clearing old maps before processing fresh ones individually seems to help.

What is actually going wrong?
The pattern is fairly simple.
You use a Pint-Sized Phantom Map, get the grave objective, complete it, then use another map.
And nothing happens.
The next quest doesn't properly kick in.
This appears to be affecting some players repeatedly, while others seem to wander happily through Appalachia digging up corpses as though Bethesda intended this to be a normal Tuesday afternoon.
Server hopping has been the dependable escape hatch. Leave your world, join another one, activate the map again and the quest can start working.
It works.
It's also ridiculous.
When the official solution to a treasure map is effectively leave reality and try another reality, we've moved beyond troubleshooting and into Doctor Who territory.
Try the one-map-at-a-time workaround first
The more interesting workaround is to stop hoarding Phantom Maps altogether.
Players have independently reported success by clearing out the maps they already had, getting a new one and using that one straight away.
So the process becomes:
Remove old Phantom Maps from your inventory.
Earn a new Phantom Map.
Activate it immediately.
Complete the disturbed grave.
Get the next map.
Repeat.
Do not build yourself a lovely little stack of twelve maps and think: I'll do those later.
Apparently the serial killer investigation has adopted the organisational principles of supermarket yoghurt. One at a time, consume immediately, do not store beside similar products.
What makes this workaround worth trying is that players have managed to repeat the process several times without having to server hop between every grave.
That doesn't prove why it works.
It just means that, at the moment, keeping the map cycle clean seems less likely to make Fallout 76 stare blankly at you and forget what a quest is.
Are stacked Phantom Maps causing the bug?
We don't know.
And this is where it's very easy to turn a useful workaround into fake certainty.
Several players finding that removing old maps helps does not automatically mean multiple maps in your inventory are the root cause.
There could be some quest-state nonsense involved. It could be related to old map items. It could be server state. It could be three separate systems held together with a paperclip somewhere beneath West Virginia.
Until Bethesda confirms what's happening, the honest version is simply:
Clearing old maps and using newly earned maps one at a time is currently helping some players avoid the problem.
That's useful.

What if that still doesn't work?
Server hop.
At the moment, that's still the fallback I'd trust most.
Leave your current world, join another server and try the map again.
If it starts working, carry on. If you want to reduce the chance of ending up back here five minutes later, process your next maps individually rather than letting them accumulate.
And if a map refuses to trigger anything, I wouldn't keep feeding more maps into it hoping Fallout suddenly develops a sense of responsibility.
Stop.
Change server.
Try again.
